I think the movie Star Wars hits the largest number, but if we're only considering games..... Baldur's Gate II?

Nah. Austin Powers went out of it's way to hit as many as possible for a movie.

For a game...I'm thinking Ultima. The Guardian just kept egging the Avatar on each game until he finally got tagged by the now uber-hero. A little well-placed smacking early in the game and the Guardian would have been safe.
